Contractor poured 6 inches of concrete over my tie-in plates on the Shell job

Went back to a joint I fit up in September for the hydro test and the concrete guys had buried my weld access holes, took me 2 hours with a chipping gun to get back to the plates, has anyone else dealt with crews ignoring your marked-out openings?

Buried my weld access holes" is crazy, did you have the plates painted or flagged with anything bright before you left? Cause if not, I get why they might've missed it, but 2 hours of chipping is still total BS on their part.
