Heard a foreman say "just tack it" for a 6-inch steam line, walked away before I said something
That 6-inch steam line carries 150 psi and they wanted a quick tack to save 2 hours before the shutdown window closed, has anyone else seen a shortcut like that actually hold up on inspection?

Back in 2016 I watched a crew tack a 4 inch glycol line to beat a turnaround deadline, same kind of deal. They put two tacks on it thinking it would hold until the next morning, but when the hydrotest came around the next shift, it blew right at one of those tacks and sprayed hot glycol everywhere. Luckily nobody got hurt, but that line had to be cut out and redone anyway, costing way more than the two hours they saved. I learned then that if you see a tack on anything bigger than 2 inches, you speak up right away or you find another spot to work that day.
