Shop lead keeps passing tube welds that never got a root pass, and it's gonna bite someone
Three weeks ago on a boiler rebuild outside Gary, Indiana I got called in to fix leaks on a bank of tubes another crew had already signed off on. Started grinding one out and there was no root pass at all, just a cap laid over the gap, and I ended up finding the same thing on 6 more before I quit counting. The shop lead there was pushing guys to hit a tube count per shift and the paperwork got signed without anyone actually looking inside the joint. I know because I kept the cut-out sections and showed them to the QC guy, who went white. If you're on a job where guys get rewarded for speed over fit-up, start cutting a sample before you trust a sign-off, and ask yourself how many of those welds are already under pressure.
