Took me 18 months to notice our daily huddle notes were a total mess
Our field supervisors were typing progress updates into separate Excel sheets and nobody caught it until the PM tried to pull a weekly report and found three different versions of the same building floor. Has anyone else dealt with a tool change that just made everyone go rogue with their own spreadsheets?

Three different versions of the same floor sounds like a nightmare waiting to happen. Was the tool change something that got pushed down from higher up, or did the team actually get a say in picking it? Because that usually makes or breaks adoption. I've seen it happen so many times where management picks some expensive software and everyone just nods along, then quietly goes back to what works for them. Did your PM ever figure out who started the spreadsheet habit first, or was it a slow spread across the team? That kind of info could help untangle why it got so far off track.

The tool change was actually pushed from the team up, not the other way. A couple guys on the night shift got tired of fixing the same mistakes over and over so they started using this free open source thing and it just worked better. Nobody from above even noticed for like 3 months until the PM saw the numbers go up.
